Last Will and Testament
The official Michigan Statutory Will form, set out in Michigan law (MCL 700.2519). Fill in the blanks to name who receives your property, a personal representative, and a guardian for minor children.
- Must be signed by you and two adult witnesses
- Witnesses should not be people who inherit under the will
- Read the notice and instructions before filling it out

Health Care Power of Attorney
Michigan calls this a Durable Power of Attorney for Health Care, or Patient Advocate Designation. It lets you name a trusted person to make medical decisions if you cannot.
- Sign in front of two qualified adult witnesses
- Your patient advocate must sign the acceptance section
- Give copies to your doctor and family

Living Will
